Absence
The Headteacher is no longer allowed to authorise holidays requests, she will look at requests for leave of absence from school in exceptional cicrumstances. If you would like to apply for absence from school for exceptional circumstances please complete this form two weeks prior to the absence.
Absence Form
Illness
If your child is off sick for any reason we expect a phone call into school on the first day of absence before 9.15am and each subsequent day of absence, on return to school you will need to complete a reason for absence form. If you do not complete this form the absence may go down as unauthorised.

If you have a change of contact details or need to inform us of any new information regarding your child please complete a change of details form.

Walking Home
If you would like your child to be able to walk/cycle home alone we need permission for this to happen. We recommend that only children in Year 5 and Year 6 are to be allowed to walk/cycle home alone and only if they know the route well, are responsible and know where to go and what to do in case of an emergency.
Pupil Leaving School
If your child(ren) is due to leave the school outside of the normal stage ie before end of Year 6 then please ensure that you complete a Pupil Leavers Form before your child's last day at St Norbert's.The school must be informed as soon as you have made the decision to leave as we have to inform Lincolnshire County Council Admissions and make preparation for your child's pupil file to be sent on to his/her new school.
